Discharge Procedure

WWHS Policy Statement:

Discharge planning enables efficient and effective transition of patients from hospital to the community and where required, facilitates continued care through arrangement of community services. Effective discharge is a critical component of patient/resident management to ensure appropriate utilisation of resources

Discharge planning commences at pre-admission clinic or on admission to the emergency department or the ward, resulting in a seamless continuum of care from pre-admission, through acute and sub-acute to discharge back to the community

Objective

  • To aim for discharge by 10:00 hours. This shall occur in consultation with all key stakeholders and transport plans made accordingly.
  • To ensure discharge processes meet the needs of patients/residents and their families and facilitate optimal care delivery that is safe and efficient.
  • To ensure the multi-disciplinary team provides a safe, successful and efficient discharge of patients/residents from acute to sub-acute, to community facility or to home, with or without community support.
  • To ensure clinical staff have the appropriate assessment and communication skills necessary to facilitate discharge.
  • To ensure care and service delivery is provided in a seamless manner from pre-admission to discharge and ongoing patient management.
  • To ensure all aspects of the discharge planning process at the Hospital adhere to legislative criteria requirements, the values of Hospital and evidence based practice.
