INTRODUCTION

Aged Care is, and always has been, a priority area of the West Wimmera Health Service. The quality, spaciousness and comfort of our Nursing Homes and Hostel units are excellent by any measure and supported by highly professional, compassionate and caring staff.

West Wimmera Health Service offers quality aged care low and high care facilities in 5 West Wimmera towns in Victoria.

Low Care Residential Facilities

Low level of care in a hostel situation is provided in:

In Natimuk, Lockwood Hostel provides 11 dementia specific units.

High Care Residential Facilities

High level of care in a nursing home situation is provided in:

In Nhill, Iona Digby Harris Home provides 8 dementia specific beds and 6 Psycho-geriatric beds.

 

Aged Care Application Requirements

All people requesting entry into either low or high care must be assessed by an Aged Care Assessment Service (ACAS) representative – which is valid for 12 months.

Anyone can request an assessment, however if you wish arrangements can be made through your local doctor or heath service.

A copy of the current ACAT form (Aged Care Client Record) must be provided to the facility upon admission.

 


NHILL

 

IonaDigby Harris Home

IONA Digby Harris Home is situated beside the West Wimmera Health Service – The Nhill Hospital.

 

The home is a twenty six-bed ground level facility with easy access via covered entrance at the front of the building.  There are three purpose built units within the facility ensuring residents are cared for in a manner specific to their needs.  Each unit has an easily accessible enclosed garden.  Security locks are in place on the main external door.  There are two, two bedroom units in the Frail Aged wing; all remaining rooms are single with shared ensuite.  Residents have a nurse call bell located beside their bed and in each ensuite.  There are separate kitchen, dining room and sitting room access for use by residents and visitors in each unit.

 

Each room is furnished and residents are encouraged to decorate their own rooms with belongings and mementos of their life, according to the agreed standard with the Service. Every effort is made to make the communal areas pleasant and homely.  Physical activity is encouraged where possible and residents can make appropriate use of the facilities of the Home and those in the locality.

For a healthy life both mind and body need to be cared for.  We take care that the quality of food served and the choice, preparation and presentation of meals is appealing, nourishing and nutritional to residents.  Relatives and friends are encouraged to dine with the residents for a small fee.  Special dietary requirements are catered for and consultation from a dietician is provided.

 

IONA Digby Harris Home has close liaison with the staff of Allied Health so physiotherapy, podiatry, occupational therapy, speech pathology, dietician, and dentist are all on call to service the residents if the need arises.  The continence nurse and diabetes educator are also available.

There is a regular visit by the hairdresser.

The residents may participate in any of the activities run by our Day Centre.

Religious services are arranged at the Home to meet the diverse range of religious practices of our residents.  Residents can also attend the service conducted in the Day Centre on Friday mornings.

 

Resident Admission

All prospective residents are requested to contact the Operations Manager in the first instance of contemplating Residential Aged Care.  West Wimmera Health Service maintains an Admission List which determines who is offered a bed when a vacancy arises. 

In order to be placed on the Admission List, a person must firstly have a current ACAS Assessment.  They will then be asked to complete an admission form, provide a copy of the Aged Care Client Record (from the ACAS Assessment), a copy of Powers of Attorney and an Asset Assessment.

An appointment with a Social Worker can be arranged to assist with the completion of any paperwork and to discuss the whole range of issues associated with such a change in lifestyle.

It is desirable for prospective residents and their family representatives visit their chosen facility prior to admission.  This provides an opportunity to view the home, meet the staff and discuss with the Director of Nursing any concerns and queries regarding prospective care. 

Admission is on a needs basis, determined with the prospective resident, their family or friends, their General Practitioner, the Executive Director of Aged Care and the Director of Nursing/Unit Manager.

JEPARIT

Jeparit & District Nursing Home

Jeparit is situated approximately 30 minutes drive from West Wimmera Health Service Nhill Hospital and 45 minutes drive from Horsham.

 

Jeparit & District Nursing Home is a 15 bed aged care facility that encompasses 5 Low Care beds and 10 High Care beds.  In October 2001 we reopened our open plan facility after a 21-month redevelopment.

The Low Care facility has 5 single rooms with ensuites and a kitchenette with a Dining/living area.  Three of the rooms face the residents wandering area and two rooms face into the courtyard.

In the High Care area we have six single rooms that have shared or separate ensuites.  We have two double rooms which each have their own ensuite. Located in the centre of the facility are the High Care Residents living and dining area, and residents also use the Day Centre for meals on days when this is operational. 

We have a quiet lounge area available for residents family and friends use for special occasions/personal use.

RAINBOW

Rainbow is situated approximately 45 minutes drive from West Wimmera Health Service Nhill Hospital and an hour and twenty minutes drive from Horsham.

Rainbow Nursing Home

Rainbow Nursing Home is a ten-bed ground level facility with easy access.  Security locks are in place on the main external door.  There are two shared bedrooms, and all remaining rooms (six) are single.  All rooms have ensuites and a bathroom is available for those residents who prefer a bath.

Residents have a nurse call bell located beside their bed and in each bathroom.

Rainbow Hostel

Rainbow Hostel adjoins Rainbow Nursing Home and Rainbow Hospital

The Hostel is a ten bed ground level facility with easy access.  All residents occupy single bedrooms with a private ensuite.  There are facilities for couple accommodation. 

Residents have nurse call bells located at their bedside, in their ensuite and in communal areas.

The Nursing Home and Hostel Facilities

Each room is furnished and residents are encouraged to decorate their own rooms with belongings and mementos of their life, according to the agreed standard with the Service. Every effort is made to make the communal areas pleasant and homely.  Physical activity is encouraged where possible and residents can make appropriate use of the facilities of the Home/Hostel and those in the locality.

For a healthy life both mind and body need to be cared for.  We take care that the quality of food served and the choice, preparation and presentation of meals is appealing, nourishing and nutritional to residents.  Relatives and friends are encouraged to dine with the residents for a small fee.  Special dietary requirements are catered for and consultation from a dietician is provided.

The residents may participate in any of the activities run by our Day Centre.  A calendar of activities is posted on the notice board each month.

Religious services are arranged at the Home to meet the diverse range of religious practices of our residents.  Residents can also attend the service conducted in the Day Centre on Thursday afternoons.

KANIVA

Kaniva is situated on the Western Highway approximately 25 minutes drive from West Wimmera Health Service Nhill Hospital and 75 minutes drive from Horsham.

Kaniva Nursing Home

Kaniva Nursing Home is an eleven-bed ground level facility adjoining the Kaniva Hospital, with easy access via covered entrance at the front of the building.  Each room has sliding door access to our securely enclosed beautiful garden.  Security locks are in place on the main external door.  All rooms are single with ensuite.  Residents have a nurse call bell located beside their bed and in each bathroom.

Each room is furnished and residents are encouraged to decorate their own rooms with belongings and mementos of their life, according to the agreed standard with the Service. Every effort is made to make the communal areas pleasant and homely.  Physical activity is encouraged where possible and residents can make appropriate use of the facilities of the Nursing Home and those in the locality.

Kaniva Hostel

Situated in Arthur Vivian Close, the Kaniva Hostel is part of the Kaniva Cottages and consists of self-contained two bedroom Cottages, with front and back yards and an enclosed garage under the main roof.  Each cottage has a reverse cycle air conditioner providing both heating and cooling.  Each unit is furnished with a bed and mattress, bedside tables, dining, lounge and recreational furnishings.

Bed linen, blankets, absorbent or waterproof sheeting, bath towels, face washers, soap and toilet paper are also provided.  Residents have nurse call bells located in their ensuites and living areas.

Because of the uniqueness of the Kaniva Hostel many residents choose to bring their own furnishings, belongings and mementos when they move in.  This is actively encouraged as we believe this will more quickly ensure that the Cottage becomes your home.

The Kaniva Cottages complex also comprises rental units and license to occupy units.

Kaniva Hostel and Nursing Home has close liaison with the staff of Allied Health so physiotherapy, podiatry, occupational therapy, speech pathology, dietician, and dentist are all on call to service the residents if the need arises.  The continence nurse and diabetes educator are also available.

There is a regular visit by the hairdresser at the Kaniva Hospital/Nursing Home.

The residents may participate in any of the activities run by our Day Centre. 

Religious services are arranged at the Home to meet the diverse range of religious practices of our residents.  Residents can also attend the service conducted in the Day Centre on Tuesday.   Hostel Residents can be picked up by the Day Centre bus to attend the service, or any other Day Centre activities.

NATIMUK

Natimuk is situated approximately 50 minutes drive from West Wimmera Health Service Nhill Hospital and 10 minutes drive from Horsham.  The Natimuk Nursing Home, Alan W Lockwood House Hostel and Trescowthick House Hostel are co-joined to create a fabulous, all encompassing facility.

Natimuk Nursing Home

The home is a twenty (20) bed ground level facility with easy access via covered entrance at the front of the building.  The unit has an easily accessible enclosed garden.  Security locks are in place on the main external door.  Our recently renovated nursing home offers our residents absolutely state-of-the-art aged residential care.

Alan W Lockwood Special Care Hostel

Lockwood Hostel is a dementia specific hostel and provides safe and secure accommodation for residents with this condition.

The Hostel is an eleven-bed ground level facility with easy access via a covered entrance at the front of the building.  All rooms are single with shared ensuite.  Residents have a nurse call bell located beside their bed and also in each ensuite. 

Trescowthick House Hostel

Trescowthick Hostel is a ten-bed ground level facility with easy access via a covered entrance at the front of the building.  The unit has easy access to an enclosed garden.  Security locks are in place on the main external door.  All rooms are single with shared ensuite.  Residents have a nurse call bell located beside their bed and also in each ensuite.
