Community Nursing

Community Nursing (formally known as District Nursing) is provided by qualified and experienced Registered and Enrolled nurses. Community Nurses provide services either at our sites (in-Centre) or visit the homes of people in the communities across our catchment (in-Home). They provide a range of healthcare services including dressing wounds, checking blood pressure, and taking bloods.

What we offer

Our Community Nurses help people during periods of illness or during rehabilitation. We aim to promote independence by teaching clients and their families the skills to care for themselves in the comfort and safety of their own homes.

The days and times that our nurses will visit your home are negotiated to suit both staff and clients. Nurses do not provide domestic assistance such as cleaning; however appropriate referrals can be made for an assessment if required.

Community Nurses also:

  • provide information about additional services available, and with the permission of the client, can make referrals to other services as required.

  • organise any equipment required to make the client safe and comfortable in their home.

How to access

We can arrange for Community Nurses to visit you following a referral from a GP, hospital, Community Health Centre, My Aged Care, a family member with the client's consent, or by self referral.

All clients over the age of 65 are required to be registered with My Aged Care and have a Nursing Referral Code.

If the client's environment is considered to be unsafe, for any reason, staff have the right to decline a visit until the issue has been resolved. This includes securing animals when staff visit and not smoking within the house for the duration of the visit.

Nursing - in Centre (per visit)

Concession - $4

Non-concession - $8

Nursing - in Home (per hour)

Concession - $6

Non-concession - $15
