Support At Home Packages

Support At Home Packages are for people aged 65 years or older (or 50 years or older for Aboriginal or Torres Strait Islander people) who need a little more assistance to maintain their independence living at home. With a focus on personalised care, they cater to a wide range of needs, including assistance with daily tasks, personal care, medication management, minor home modifications, allied health and nursing support.

A Support At Home Package is a government funded package that provides assistance accessing and paying for some of the extra health and living needs as people get older. It’s delivered in-home and is designed to help enable people to remain living as independently as possible in their own home.

What we offer

Care and services available under a Support At Home Package include:

Aids and Equipment - the provision of aids and equipment to assist to perform daily living tasks, mobility and communication, including safety monitoring and alarms.
Allied Health and Therapy Services - healthcare services provided by professionals such as a physiotherapist, occupational therapist, speech pathologist, podiatrist, dietitian, exercise physiologist, and social worker. This also includes hydrotherapy.
Care and Package Management - ongoing assessment, planning and care coordination and ongoing administration and organisational activities for smooth delivery and management of a package.
Continence Nurse and Aids - specialised assistance and resources to manage continence issues and promote bladder and bowel health.
Domestic Assistance - assistance with household tasks such as cleaning, laundry, shopping and general household tasks.
Home and Garden Maintenance - basic home maintenance such as cleaning gutters and light gardening to maintain a safe home environment.
Minor Home Modifications - adaptations and alterations made to the home to improve accessibility and safety.
Nursing - nursing, clinical care, palliative care services either provided to you in the comfort of your own home or delivered within our West Wimmera Health Service facilities.
Personal care - assistance with daily living and self-care tasks like bathing, dressing, grooming, medication (including webster packs), shopping and assistance with meals (including partially funded Meals on Wheels).
Respite - support services designed to provide temporary relief and support for caregivers.
Social Support - Group - group activities and socialisation opportunities held at your local centre and off-site destinations. Transport is provided as part of the program plus meals are supplied for all centre-based activities.
Social Support - Individual - one-on-one support and companionship provided in the home or community for example spending time with you in your home or out and about for leisure or attending appointments and paying bills.
Transport - assistance with transportation to appointments, social activities, and usual outings.

How to access

To access Support at Home packages you need to contact My Aged Care on 1800 200 422 and request an assessment. They will create a referral to us and we will take it from there. Our staff are here to support you with this step, so please call us and we can start the process together.

Cost

Under the program, the government pays the cost of clinical supports such as nursing care and allied health services. For non-clinical services like domestic assistance, personal care, transport, or social support, people may be asked to contribute a portion of the cost depending on their income and assets. These personal contributions help cover day-to-day living support while ensuring access to essential health and wellbeing services.
